WHAT INFORMATION DO I NEED TO PROVIDE FOR THE PREPARATION OF MY BANKRUPTCY?

Upon receipt of the initial down payment, we will send you a questionnaire to complete and ask you to provide the following documents to our office:

1. All paystubs or proof of income received during the previous six month period

2. Federal and state income tax returns from the last two years

3. Previous three months bank statements

4. Copies of all vehicle titles and deeds to real property

5. Most recent billing statement or invoice from all creditors that are not included in your credit report.

 
 
 

We will also need your date of birth and SSN before we can pull your credit report. Once I have received the DOBs and SSNs, we will forward your credit report. Please note that we cannot prepare your bankruptcy petition until we have received all the documents and your answers to the questionnaire.
